Bridging Safety and Well-being: Why Total Worker Health® is the Future of EHS

For decades, the standard for excellence in occupational safety was defined by the absence of injury. If the hard hats were on and the guards were in place, the job was considered “safe.” However, as the boundaries between work and life blur and the realization that safety requires more than physical safety requirements, occupational safety and health is shifting toward a more holistic model. NIOSH’s Total Worker Health (TWH) is such a model that embraces all elements of safety and health.

We’ve recently blogged on TWH (Cultivate a Thriving Workforce) AND STAR (OSHA VPP/STAR), but for organizations already committed to high-level safety cultures—such as those pursuing or maintaining OSHA VPP/STAR status—TWH isn’t a replacement; it’s the logical next step in evolution.

What is Total Worker Health®?

Total Worker Health® is defined by NIOSH as policies, programs, and practices that integrate protection from work-related safety and health hazards with the promotion of injury and illness prevention efforts to advance worker well-being.

Traditional safety focuses on what happens to individuals at work. TWH expands that focus to include how work affects employee total health, including:

  • Work-related stress and mental health.
  • Shift work and sleep deprivation.
  • Economic security and living wages.
  • Chronic disease management (for example, diabetes, hypertension) as it relates to workplace factors.

And have no doubt that employee total health will affect their work, safety, and that of their team.

The Power Couple: OSHA VPP and TWH

Several of our clients ask, “We are already a VPP Star site; why do we need TWH?” The answer lies in how these two frameworks complement each other.

OSHA VPP (Voluntary Protection Programs) is the gold standard for Traditional Safety. It relies on four pillars:

  • Management Leadership,
  • Employee Involvement,
  • Worksite Analysis, and
  • Hazard Prevention/Control

Total Worker Health® acts as the Multi-Tool or Multi-Dimension Lens that enhances the pillars. Total Worker Health embraces a framework of 10 essential elements:

  • Prevention and Control of Hazards
  • Build Environment Supports
  • Community Supports
  • Compensation and Benefits
  • Healthy Leadership
  • Organization of Work
  • Policies
  • Technology
  • Work Arrangements
  • Workforce Demographics

Here are a few examples of how TWH and VPP align:

VPP ElementTWH EnhancementThe Combined Result
Employee InvolvementAddresses “off-the-clock” stressors (finances, family care) that lead to “on-the-clock” distractions.A more focused, present, and engaged workforce.
Worksite AnalysisLooks beyond physical hazards to “Psychosocial Hazards” like burnout and workplace incivilities.Identification of the root causes of human error to include all elements of safety and fatigue-related incidents.
Hazard PreventionIntegrates wellness (e.g., ergonomic workstations combined with movement breaks and nutrition).Reduced rates of musculoskeletal disorders (MSDs) and faster recovery times.
Management LeadershipLeaders demonstrate care for the human, not just the worker.Higher retention, improved morale, and a true “culture of care.”

Why EHS Leaders are Making the Switch

Integrating TWH into your existing safety management system offers a competitive edge that goes beyond compliance:

  1. Potentially Lowering Healthcare Costs: By addressing chronic conditions and mental health through the workplace, companies see a direct reduction in insurance premiums and workers’ compensation claims.
  2. Combating the “Great Resignation”: Employees are increasingly choosing workplaces that prioritize their mental and physical well-being. TWH is a powerful recruitment and retention tool.
  3. Cultivating Well-Being: As we blogged before, Mark Crowley, emphasizes that, “…employee well-being isn’t a nice-to-have, it is a direct, consistent, and powerful force for team and organizational success. Research from Oxford University and others proves that leaders who prioritize well-being create environments where employees not only thrive but outperform.”
